To download the transcript request forms, click on the link for either college or high school as appropriate.

Complete each form by filling out the address of the school from which you are requesting the transcript and attach a check made out to that school in the amount of the transcript fee.

NOTE: McKendree University does not have the address and fee information for your colleges or high school. You will have to contact those institutions to get their transcript request address and fee schedule.
